Patient education, care planning and giving patients access to a team of specialists have contributed to NHS Newham Clinical Commissioning Group (CCG) being rated as ‘Outstanding’ for its diabetes care following a 2016/17 assessment by NHS England.
Newham CCG commissions most local health services and all GP practices in the borough.
Diabetes is a condition where the amount of glucose in a person’s blood is too high because their body cannot produce enough insulin, or where the insulin they produce does not work properly.
While type 1 diabetes cannot be prevented, type 2 diabetes is largely preventable through lifestyle changes. In Newham, more than 26,500 people are diagnosed with type 2 diabetes. This accounts for nearly 10 per cent of adults in the borough.
Newham’s success can be attributed to a number of initiatives such as increased provision of structured education sessions as a result of patient feedback, including when and where sessions are held.
As well as classroom-based learning, Newham’s community prescription work has also been at the heart of tackling diabetes in the borough, with eligible patients enrolled in free activity sessions through a variety of schemes such as walking football, tai chi and chair-based exercise.
Every patient is also given an individual care plan, with a team of specialists available to support them with their care. In addition, GPs are sharing learning with each other across the borough.
The 2016/17 diabetes rating is based on two key diabetes indicators:
Dr Prakash Chandra, Chair of NHS Newham CCG, said: “We are delighted with this outstanding result for Newham. The rating recognises the great work by GPs, practices and all members of staff involved in diabetes care in the borough.
“We know that high rates of type 2 diabetes are a big issue in Newham, with over 1,800 new diagnoses a year and one in eight residents at risk of developing it in the future.
“To reduce your risk of developing type 2 diabetes, the best thing you can do is to eat healthily and increase the amount of activity you do. Both of these will help with weight management including weight loss if required.”
