What is the Newham self-management support programme?

Self-management is a term that covers a whole range of things you can do to help yourself live a better life, which is especially important when you have a long-term health condition.

The Newham self-management support programme is a new, free, service being provided by local pharmacists to help you manage your long term conditions better and get more out of life.

Local, Newham-based, pharmacists’ have been trained to support you to identify what you can do to improve your health and wellbeing.

Through one-to-one coaching sessions, you will be supported to develop a personalised action plan that will help you to improve your health and wellbeing by making changes to your lifestyle to achieve the results you want to see.

What does the programme involve?

The self-management support programme consists of three sessions with a trained pharmacist over a 12 week period where they will support you to identify changes that will help you better manage your long term conditions.

You will be supported to develop a personalised wellbeing plan, which will describe the activities; lifestyle and behaviour changes that you wish to adopt to better manage your long term conditions. You will also be shown the Directory of Services so that you can look at the services/classes that best suit your needs to help achieve the changes you want to make.

How will self-management support help me?

Supported by your pharmacist, you will learn how to better control your conditions and identify local services and projects that you can access to achieve the changes you want to make.

There are significant benefits to making positive lifestyle choices. For example being active is really good for your mind as well as your body. Getting some physical activity each day can help prevent you getting depression, anxiety and other mental health problems.

Lifestyle changes such as stopping smoking, eating healthy food and losing weight can make a huge difference to your health and wellbeing. These changes can help you live a healthier and happier life.

How do I sign up?

You will need a referral from your GP to access the service. If you have long-term conditions and think this could help you please talk to your GP. You can also call any of the participating pharmacies listed below to ask for more details about the service.

What is the Newham Self-Management Support Programme (SMSP)?

The Self-Management Support Programme is a local service in Newham provided by community pharmacists, which helps and supports patients with long term conditions to consider the activities, lifestyle and behaviour that they want to adopt to manage their long term condition(s) better and identify local services which can support the activities and changes identified.

People taking part will be supported by a trained pharmacist over a 12 week period through one-to-one coaching sessions to develop a personalised action plan, known as a wellbeing plan that will identify lifestyle changes that will help to improve health and wellbeing.

Who can access the service?

The service is available to all eligible patients who are registered with a General Practitioner (GP) in Newham who have 2 or more of the following long term conditions: Atrial Fibrillation, Coronary Heart Disease, Cardiovascular Disease, Chronic Obstructive Pulmonary Disease (COPD); Asthma; Stable Hypertension; Controlled Diabetes.

How can I access the service?

How many times do I need to meet with my SMSP pharmacist?

The Self-Management Support Programme consists of three sessions with a trained pharmacist over a 12 week period where they will support you to identify the changes that will help you better manage your long term conditions.

What is a wellbeing plan and how will this help me?

A wellbeing plan is a personalised action plan developed and driven by you with the support of your pharmacist acting as your wellbeing plan partner. Your wellbeing plan will describe the activities; lifestyle and behaviour changes that you wish to adopt to better manage your long term conditions. You will also be shown the Directory of Services so that you can look at the services / classes that best suit your needs to help achieve the changes you want to make

Who will help me to complete my wellbeing plan?

You will be supported to complete your wellbeing plan by a trained pharmacist. SMSP pharmacists have received significant training in health coaching and motivational interviewing.  Through one-to-one coaching sessions, your pharmacist will help you to develop a personalised wellbeing plan that will help you improve your health and wellbeing.

What are the opening hours of my SMSP pharmacy?

Opening times vary across SMSP pharmacies with most open six days a week and several which are open seven days a week. You can find opening times of the pharmacy providing you with self-management support here.  You will need to contact the pharmacy of your choice to make an appointment to see a trained pharmacist.

Where can I find full contact details of my Wellbeing Plan Partner?

Full contact details will be recorded on the ‘My Personal Details’ page of your wellbeing plan under the ‘My Wellbeing Plan Partner’ section. You can also find contact details of the pharmacy providing you with self-management support here.

How will I know about available community services and resources in Newham?

Your Wellbeing plan Partner will support you to identify appropriate national and local services and projects that can support you to achieve the changes you want to make by using a number of signposting tools including the London Borough of Newham’s Directory of Health and Social Care Services.

Where will I discuss my Long Term Conditions (LTCs) with my SMSP pharmacist?

The pharmacy will have access to a private, confidential consultation room, in which conversations cannot be overheard by others in the pharmacy.

How will my personal information be used?

If you are happy to take part in the scheme the pharmacist will share some of your information back with your GP practice subject to you providing consent for them to do so.

Information provided in confidence will only be used for the purposes advised and consented to by you. You have the right to refuse / withdraw consent to information sharing at any time.

Is there a charge for this service?

There is no charge for the service. 

What happens if I miss an appointment?

The pharmacist will send you a text reminder of your appointment; please let them know if it is not convenient. If you miss an appointment, the pharmacy will contact you to offer another appointment.  Alternatively you can rearrange or book another appointment online, in pharmacy or via telephone.

What the evidence that self-management support will help me?

There is a growing body of evidence shows that supporting people with long-term conditions to self-manage their health and care can lead to significant benefits for the person, their health and better usage of health services.

What is One You?

The ‘One You’ campaign developed by Public Health England challenges everyday habits and behaviours, such as eating too much unhealthy food, drinking too much alcohol, continuing to smoke and not being active enough.

A short online health quiz is available which supports people to identify where they can make small changes. The quiz provides personalised recommendations and directs people to tools and advice created by experts to help them take action where it’s most needed. The results from the quiz will help you to develop your wellbeing plan

What is the Morisky Medication Adherence (MMAS-8)?

The Morisky Medication Adherence (MMA-8) questionnaire consists of 8 questions which provide information about a person’s medication adherence, which is the extent to which they take their prescribed medications as intended by their prescriber and other healthcare providers.

The results of the questionnaire will help your pharmacy wellbeing plan partner to ensure that they provide you with the most appropriate level of support.

What is the Patient Activation Survey and how will it help me?

The Patient Survey is a series of questions which will provide you with an insight into your confidence, skills and knowledge to manage your own health and wellbeing.   The results of the survey will help your wellbeing plan partner support you to identify areas of focus to help you make small changes to reach your own health and/or lifestyle goals.
